Admire Care provides Support Coordination and Specialist Support Coordination. We work with participants to understand and implement their NDIS plan and connect them to the best support and services available from NDIS providers and mainstream services.
Our support coordination or specialist support coordination service helps you make sense of your funding. Their knowledge and experience of a participant enable them to develop a tailored support plan around strengths to best achieve their goals.
It also enables them to develop funding plans to maximise the value of the plan and money value from providers.
Our team are able to access diverse therapy providers and combine training and experience to help you solve complex issues.
Once your plan is approved, we can help:
- • Explore activities, services, and social connections that are important to you
- • Connect you with both informal and formal supports needed to meet goals
- • Coordinate your support and follow up when your needs change
- • Address barriers to meeting your NDIS goals
- • Ensure timely delivery of assessments, reports and documentation required by NDIS
- • Work with NDIS registered providers and the NDIS Quality and Safeguards Commission as required
- • Guide you in selecting and implementing your services on your own
